PQRST

PQRSTと言えば、第一に思い浮かぶのは心電図ですね。
しかし、最近の医学生や研修医の先生達は「痛み」に関して病歴聴取する時のキーワードとしてPQRSTを利用しています。
なぜこのような話が出てきたかというと、先日、江口先生に内科のカンファレンスで症例プレゼンテーションをしてもらった時に、
痛みに関しての病歴聴取が不十分だったからです。以下のPQRSTをもれなく記載してあるカルテは滅多に見ないという話もしました。
江口先生に宿題として「痛み」に関しての病歴聴取で聴くべき項目を調べておくよう話しましたが、
その答えの一例としてブログに掲載させて頂きます。
"P"：Provoke and Palliation of the symptom（増悪因子と軽快因子）
労作で誘発される胸痛→狭心症？、油物を食べた後に頻繁に生じる右季肋部痛→胆石症？などの判断が出来ますね。
"Q"：Quality(intensity and character) （強さと特徴）圧迫される感じの胸痛→狭心症？
心筋梗塞？となりますし、ピリピリする胸痛と言えば帯状疱疹の初期？などと想像されます。
"R"：Radiation and Location（放散の有無と部位）よく心筋梗塞で下顎部が痛いとか、
心窩部が痛いとか言われますね。
"S"：Symptoms which are associated with the symptom
��関連していると思われる症状）心窩部痛を訴えている方にタール便もあったというのは大事な情報ですね。
"T"：Timing(onset,duration,and frequency)時間因子（発症起点、持続時間、頻度）
突然発症は「詰まった」「破れた」が多いとよく言われていますね。
江口先生、これらをもれなく聴いたら、それだけで鑑別診断がかなり絞れ込まれている事に気付くのではないでしょうか？
有留先生、有馬先輩は「そんなの10年前から知っていた」とか言っていませんでしたか？
PQRST勉強になります。

痛みに対する問診について、調べた限りでは、

Location(場所)/Intensity(強さ)/Quality(質)/Onset(発症様式)/Radiation(放散)/Aggravating factor(増悪因子)/Alleviating factor(改善因子)/Associated symptom(関連する症候)/
・Cloride AA(塩化物 ダブルエー)
Character(質)/Location(場所)/Onset(発症様式)/Radiation(放散)/Intensity(強さ)/Duration(持続時間)/Excerbating factor(増悪因子)/Alleviating factor(改善因子)/Associated symptom(関連する症候)/
の2つが有名そうでしたが、PQRSTの方が、憶えやすそうです。

返信削除そうやって自分で調べる事が一番勉強になりますから、気に入った方法で覚えて下さい。
症例プレゼンテーションの能力は、どこに行っても役立ちます。
先生がプレゼンテーションをした時に色々と指摘があるのは、きちんとしたプレゼンテーションが出来るようになる事を周りが期待しているからだと思って下さい。
年次が上がっていくと、あまり基本的な事は指摘してもらえなくなるので、凹まずに頑張って行きましょう。
